Job summary

Amedisys Home Health is seeking an LPN for full-time day shifts to provide adult home health visits in Ohio. The role includes coordinating with RNs and physicians to manage patient plans of care, performing essential nursing tasks, and documenting care delivered in the patient’s home.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to address patient needs and escalate changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs during visits.
• Review medications and perform medication reconciliation at visits.
• Perform procedures ordered by physician (e.g., Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.

Skills

• Minimum Hourly: $26.00
• Maximum Hourly: $32.00
