Job summary

Aviata at Shoal Creek in Crestview, FL is seeking a dedicated LPN to join our in-person nursing team during the Hiring Event. You will provide nursing services and contribute to individualized resident care plans.

Qualifications

• Experience in Skilled Nursing/Rehabilitation facilities preferred.

Responsibilities

• Assist in implementation of individualized treatment plans for each resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services.
• Document evaluations of residents’ medical status during admission and care.
• Other duties as assigned.

Major Responsibilities

• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.

• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment.

Qualifications
