Job summary

Aviata Health Group is hiring an LPN for the Bay location in Tampa, FL. This is a full-time night shift (7p–7a). New grads are welcome to apply and join a family/team environment focused on patient care and skill advancement.

The LPN will provide direct patient care in a skilled nursing/rehabilitation center under supervision, support daily living activities, supervise CNAs, and participate in program oversight and documentation.

Qualifications

• Must possess a current, unencumbered LPN license in state of Florida.

Responsibilities

• Provide direct nursing care to residents and patients in a skilled nursing/rehabilitation setting.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls and Pressure Sore Programs and provide nursing services.
• Document and monitor patient status during admission and treatment.

Major Responsibilities

The Licensed Practical Nurse (LPN) care giver provides direct patient care to the residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse. Responsibilities may include but are not limited to:

• Assist in the implementation of individualized treatment plans for each assigned resident.

• Supervise CNAs and participate in their evaluation.

• Oversee Falls Program and Pressure Sore Program.

• Provide nursing services.

• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment.

• Other duties as assigned.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.

• Experience in Skilled Nursing/Rehabilitation facilities preferred.

• You must be qualified, compassionate, and dedicated to a job well done.
