Job summary

Aviata Health Group is seeking a Licensed Practical Nurse (LPN) to join our West Palm Beach team. You will provide compassionate, patient-centered care to residents and patients, under the guidance of a professional nurse, and contribute to personnel evaluations and program oversight.

Qualifications

• A current, un encumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Ability to work a flexible schedule, including evenings, weekends, and holidays.

Responsibilities

• Provide direct patient care to residents and patients, assisting with activities of daily living under the direction of a professional nurse.
• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services and conduct thorough evaluations of each resident’s medical status upon admission and throughout their course of treatment.

About the Role:
