Aviata Health Group in Brooksville, FL seeks a licensed practical nurse to provide direct patient care in a skilled nursing center. You will support daily living activities, follow treatment plans, and document residents' status with a collaborative team.

The role emphasizes bedside nursing, supervision of CNAs, and participation in falls and pressure sore programs. Night shift, part-time hours, and family-friendly benefits are available.

Major Responsibilities

The Licensed Practical Nurse (LPN) care giver provides direct patient care to the residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse. Responsibilities may include but are not limited to:

• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services.
• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment.
• Other duties as assigned.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• You must be qualified, compassionate, and dedicated to a job well done.
