Aviata Health Group in West Palm Beach, FL is seeking a dedicated Licensed Practical Nurse (LPN) to join our team. You will provide direct patient care to residents, support individualized treatment plans, and collaborate with professionals to ensure high-quality, compassionate care.

You will supervise CNAs, oversee Falls and Pressure Sore Programs, conduct thorough evaluations on admission and during treatment, and contribute to a respectful, welcoming environment for residents and families.

Qualifications

• Current Florida LPN license is required
• Experience in Skilled Nursing/Rehabilitation facilities preferred
• Ability to work evenings, weekends, and holidays as needed

About the Role:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team! As a Licensed Practical Nurse, you will play a vital role in providing exceptional, person-centered care to our residents and patients.

Responsibilities:

• Provide direct patient care to residents and patients, assisting with activities of daily living under the direction of a professional nurse.
• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services and conduct thorough evaluations of each resident’s medical status upon admission and throughout their course of treatment.

Requirements:

• A current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Ability to work a flexible schedule, including evenings, weekends, and holidays.
