Job summary

Park Avenue is seeking a dedicated LPN to provide compassionate, resident-centered care in a supportive team environment. You will perform assessments, admissions, and care planning while documenting changes and coordinating with physicians.

You will lead CNAs and CMTs, ensure adherence to protocols, and respond effectively to emergencies, contributing to high-quality patient outcomes and a collaborative workplace.

Qualifications

• Active LPN license in good standing.

• Strong communication with residents, families, and team.

Responsibilities

• Conduct assessments and document changes in residents’ condition.
• Perform admission duties including MAR, TAR, POS, and medication orders.
• Coordinate discharges with physicians, guardians, and transportation.
• Provide wound care following facility protocols and dressings.
• Lead CNAs and CMTs to ensure compliance with policies.
• Respond promptly to emergencies and escalating when needed.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings




• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications




• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation




• Provide wound care: follow protocols, irrigate, pack, and remove dressings




• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies




• Respond effectively to emergency situations
