Job summary

Forest Hills Care & Rehab is seeking a Licensed Practical Nurse to join our compassionate care team in Oklahoma. The LPN will provide skilled nursing care, administer medications, and participate in care planning to meet residents’ needs.

We offer a supportive, team-oriented environment within a larger enterprise network. The ideal candidate is licensed in Oklahoma, with CPR certification and prior nursing experience.

Qualifications

• Graduate of accredited LPN or RN program and licensed in Oklahoma.
• One year of nursing experience preferred.
• Valid CPR certification.
• Ability to work cooperatively as a team member.
• Ability to communicate effectively with residents and families.
• Ability to respond quickly in emergencies.
• Maintain confidentiality.
• Knowledge of basic training techniques for clinical staff.
• Candidates should provide a working email and phone number for employer communication.

Responsibilities

• Provide nursing care that meets company and regulatory standards.
• Initiate and lead individualized nursing care plans.
• Assess and document residents' condition and needs.
• Implement physicians' orders accurately and promptly.
• Assign nursing tasks to team members based on needs and capabilities.
• Administer medications as prescribed.
• Start IVs and perform treatments for assigned residents.
• Document treatments and care provided.
• Ensure supplies are used efficiently and equipment is clean and safe.
• Perform other duties as assigned.

Currently available shifts:

• 6:00pm to 6:00am
• Sunday through Tuesday plus every other Wednesday
• Thursday through Saturday plus every other Wednesday

Essential Functions of Licensed Practical Nurse (LPN):

• Highly motivated and able to assist in all aspects of nursing
• The candidate will provide services that exceed company and regulatory standards
• Initiates and leads individualized nursing care plans
• Assess and documents the resident's condition and nursing needs
• Accurately and promptly implements physicians' orders
• Assigns nursing care to team members in accordance with the resident's needs and the person's capabilities and qualifications
• Administers medications
• Starts IVs and performs treatments for assigned residents
• Documents that treatment as required
• Ensures that supplies are utilized economically, and that equipment is clean and maintained a safe manner
• Other duties as assigned

Qualifications of Licensed Practical Nurse (LPN):

• Graduate of accredited LPN or RN School of Nursing
• One year of nursing experience is preferred
• Valid RN or LPN license for the state of Oklahoma is required
• Valid CPR certification
• Ability to work cooperatively as a member of a team
• Ability to communicate effectively with residents and their family members, and at all levels of the organization
• Ability to react decisively and quickly in emergency situations
• Ability to maintain confidentiality
• Knowledge of training techniques for clinical staff
• Candidates must maintain a working email address and phone number for employer communication.
