Job summary

Cleveland Care & Rehab in Oklahoma is seeking a compassionate Licensed Practical Nurse (LPN) to join our team. The role covers nursing care for residents, executing physicians' orders, administering medications, and coordinating care with a dedicated team.

We offer a 12-hour shift schedule, sign-on bonus up to $5,000 for full-time, PTO after 30 days, holidays with premium pay, tuition reimbursement up to $5,000 per year, and a 401(k) with company match after one year.

Qualifications

• Graduate of accredited LPN School of Nursing.
• One year of nursing experience preferred.
• Valid LPN license for Oklahoma is required.
• Valid CPR certification.
• Ability to communicate effectively with residents and family members.
• Ability to react decisively in emergency situations.

Responsibilities

• Provide services that meet and exceed standards of care.
• Initiate and lead individualized nursing care plans.
• Assess and document residents’ condition and needs.
• Promptly implement physicians’ orders.
• Assign nursing care to team members per needs and capabilities.
• Administer medications and monitor effects.
• Start IVs and perform treatments as assigned.
• Maintain supplies and equipment in a safe, economical manner.
• Other duties as assigned.

Current shift availability-

• 6pm to 6am
• Thursday through Saturday, every other Wednesday
• Sunday through Tuesday, every other Wednesday

Essential Functions of Licensed Practical Nurse (LPN)

• Highly motivated and able to assist in all aspect of nursing
• The candidate will provide services that exceed company and regulatory standards
• Initiates and leads individualized nursing care plans
• Assesses and documents the resident’s condition and nursing needs
• Accurately and promptly implements physicians’ orders
• Assigns nursing care to team members in accordance with the resident’s needs and the person’s capabilities and qualifications
• Administers medications
• Starts IVs and performs treatments for assigned residents
• Documents that treatment as required
• Ensures that supplies are utilized economically, and that equipment is clean and maintained a safe manner
• Others duties as assigned

Qualifications of Licensed Practical Nurse (LPN)

• Graduate of accredited LPN School of Nursing
• One year of nursing experience is preferred
• Valid LPN license for Oklahoma is required
• Valid CPR certification
• Candidates must maintain a working email address and phone number for employer communication.
• Ability to work cooperatively as a member of a team
• Ability to communicate effectively with residents and their family members, and at all levels of the organization
• Ability to react decisively and quickly in emergency situations
• Ability to maintain confidentiality
• Knowledge of training techniques for clinical staff
