Job summary

Claremore Skilled Nursing & Therapy is seeking a Licensed Practical Nurse (LPN) to join our supportive team in Claremore, OK. The role features 12-hour night shifts from 6 pm to 6 am with 3-4 days off per week and differential pay.

You will administer medications, monitor residents, take vital signs, adjust care plans, and communicate changes to families and physicians. We offer competitive pay and benefits including medical, dental, vision, a 401(k) with match, ESOP, and training opportunities.

Qualifications

• Current Oklahoma LPN license in good standing.
• 1+ year experience in Skilled Nursing or Long Term Care.

Responsibilities

• Administer prescribed medications and keep residents' charts current.
• Observe residents, record changes, and notify on significant events.
• Take vital signs and assess condition.
• Update families and physicians on changes in condition or health.
• Modify care plans as conditions change.
• Prepare and administer injections and enemas.
• Monitor catheters and collect samples.
• Provide wound care.

You will have 3-4 days off per week with 12-hour rotating shifts.

6 pm - 6 am

Week 1: Mon, Tue, Sat*, Sun*

Week 2: Wed, Thu, Fri

Responsibilities and Purpose

• Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques. Ensure residents’ chart is current
• Observe residents and record significant conditions and reactions. Notify appropriate parties of residents’ change in condition and reaction to drugs, treatments and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Keep residents’ family and physician informed of any changes in resident behavior, medications or conditions of health.
• Modify residents’ care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents’ changes in condition.
• Prepare and administer injections and/or enemas.
• Collect samples and monitor catheters.
• Provide wound care.

Required Experience and Education

• Current and unencumbered Oklahoma Licensed Practical Nursing (LPN)License.
• 1+ years of experience in a Skilled Nursing or Long Term Care environment.
