Transitions At Home for Central Wisconsin is seeking a Full-Time Licensed Practical Nurse (LPN) to join our compassionate team in Baraboo, WI. You will provide skilled nursing visits, medication management, and treatments, coordinating services under RN supervision in patients' homes.

We offer a stable full-time schedule, 40 hours/week, weekdays 8:00 am-4:30 pm with no nights or weekends, plus a competitive wage ($29-$32/hour) and benefits such as 401(k) matching, tuition reimbursement, and paid

Qualifications

• Two years nursing experience; one year in home health preferred.
• CPR certification is required or preferred depending on role.
• A valid Wisconsin Driver's License and car insurance are required.

Responsibilities

Transitions At Home, is currently seeking to hire a Full-Time compassionate and dedicated Licensed Practical Nurse (LPN) to join our team and provide exceptional care to our patients in the Baraboo area.

What does a Transitions at Home LPN do? Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full-Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Night Shifts or Weekends
• Limited on-call rotation
