Transitions At Home for Western Wisconsin is seeking a full-time Licensed Practical Nurse (LPN) to join our Madison-area team. Provide skilled nursing care during routine visits, medication management, treatments, and coordinate services in patients' homes under RN supervision.

The ideal candidate will have two years of nursing experience (one year in home health preferred), CPR certification, and a valid Wisconsin driver's license with car insurance.

Qualifications

• Two (2) years’ nursing experience, 1 year in Home Health (preferred)
• CPR Certification
• A valid Wisconsin Driver's License and Car Insurance

Responsibilities

• Provide skilled nursing care including routine visits, medication management, and treatments.
• Coordinate services, counsel the patient and family in their home according to the plan of care.
• Perform duties under the direction and supervision of the RN Clinical Manager

Transitions At Home, is currently seeking to hire a Full-Time compassionate and dedicated Licensed Practical Nurse (LPN) to join our team and provide exceptional care to our patients in the Madison area.

What does a Transitions at Home LPN do?
Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full-Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Night Shifts or Weekends
• Limited on-call rotation
