Transitions At Home in Janesville is seeking a compassionate, full-time Licensed Practical/Vocational Nurse (LPN) to join our home health team in the Janesville area.

Responsibilities include skilled nursing visits, medication management, treatments, and coordinating services in patients' homes under RN supervision. Requires Wisconsin LPN license, CPR, 2+ years nursing experience (1 year home health preferred), and a valid Wisconsin driver’s license.

Qualifications

• Active Wisconsin LPN license.
• Two years nursing experience; one year in home health preferred.
• CPR certification required.
• Valid Wisconsin driver license and car insurance.

Responsibilities

Transitions At Home, is currently seeking to hire a compassionate and dependable full‑time Licensed Practical/Vocational Nurse (LPN) in the Janesville area to join our home health team. What does a Transitions at Home LPN do?

Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full-Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Weekends
• Limited on-call rotation
