Transitions At Home for Central Wisconsin is seeking a Full-Time Licensed Practical Nurse (LPN) to join our compassionate team in Baraboo, WI. You will provide skilled nursing visits, medication management, and treatments, coordinating services under RN supervision in patients' homes.

We offer a stable full-time schedule, 40 hours/week, weekdays 8:00 am-4:30 pm with no nights or weekends, plus a competitive wage ($29-$32/hour) and benefits such as 401(k) matching, tuition reimbursement, and paid

Qualifications

• Two years nursing experience; one year in home health preferred.
• CPR certification is required or preferred depending on role.
• A valid Wisconsin Driver's License and car insurance are required.

Responsibilities

• Provide skilled nursing care including routine visits and treatments.
• Manage medications and coordinate services per physician orders.
• Counsel patients and families in the home setting under RN supervision.
