Transitions At Home in Baraboo, WI is seeking a Full-Time Licensed Practical Nurse (LPN) to join our compassionate home health team. You will provide skilled nursing visits, medication management, treatments, and family counseling in patients' homes under RN supervision.

Qualifications

• Active Wisconsin LPN license.
• Two years of nursing experience; 1 year in home health preferred.
• CPR certification.

Transitions At Home, is currently seeking to hire a Full-Time compassionate and dedicated Licensed Practical Nurse (LPN) to join our team and provide exceptional care to our patients in the Baraboo area.

What does a Transitions at Home LPN do?

Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full-Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Night Shifts or Weekends
• Limited on-call rotation
