Trinity Health is seeking an experienced Licensed Practical Nurse (LPN) for home care in Michigan. This role emphasizes one-on-one patient care in the home, using advanced technology and clinical knowledge to monitor and support patients who require focused attention.

The ideal candidate will have an LPN license in Michigan, a minimum of five years in acute care (home care preferred), phlebotomy and wound care experience, and must have passed the NAPNES or an equivalent medication exam.

Minimum Qualifications:

Graduate of an accredited or state approved school of practical nursing, licensure as a LPN in the State of Michigan, a minimum of five (5) years experience in an acute care setting; home care experience preferred. Phlebotomy and wound care experience required. Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
