Job summary

Trinity Health IHA Medical Group is seeking a compassionate Licensed Practical Nurse (LPN) to support providers in delivering high-quality patient care in Michigan. You will prepare patients for visits, manage results and coordination, and assist with procedures and vaccine administration.

The role emphasizes patient education, care coordination, and collaboration within a mission-driven team dedicated to improving health outcomes. A valid MI LPN license and CPR are required.

Qualifications

• High School Diploma or GED required.
• Graduate of an accredited LPN program.
• Valid, unrestricted LPN license in Michigan; CPR certification.

Responsibilities

• Welcome and prepare patients for appointments.

• Assess if patient information requires higher level follow up.
• Collect and document patient histories, vital signs, and health information.

Trinity Health IHA Medical Group is seeking a compassionate and motivated Licensed Practical Nurse to support our providers in delivering exceptional patient care. In this role, you will prepare patients for a provider visit, provide results management support, insurance company advocacy, assist with clinical procedures, support care coordination and gaps in care and medication/vaccine administration in order to help create a positive healthcare experience every day.

What You'll Do:

• Welcome and prepare patients for appointments.

• Uses critical thinking skills within the scope of the role to assess if patient information requires higher level follow up.
• Collect and document patient histories, vital signs, and health information.
• Assist providers with exams, procedures, testing, and specimen collection.

• Performs general specimen collection duties.

General Requirements:

• EDUCATION: High School Diploma or GED and graduate of an accredited Licensed Practical Nurse Program.
• CREDENTIALS/LICENSURE : Valid, unrestricted LPN license in State of Michigan. Valid CPR certification.
• MINIMUM EXPERIENCE : 1-2 years of clinical experience in a medical or physician office. Previous nursing experience not required.
