Job summary

Trinity Health At Home – Muskegon is seeking a Home Health Licensed Practical Nurse (LPN) to provide skilled nursing care at patients' homes under the plan of care. You will be supervised by a registered nurse and operate within state practice acts, ensuring compliance with agency policies and the physician’s orders.

You will enjoy meaningful 1:1 patient relationships, opportunities for growth, and the support of nurse-led leadership.

Qualifications

• Graduate from an accredited or state approved school of practical nursing.
• Current licensure in the State of Michigan.
• CPR certification required.
• Minimum of one year experience in an acute care setting.
• Home healthcare experience preferred.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing care per the plan of care prescribed by the physician and supervising RN.
• Practice at the top of your license with autonomy and 1:1 patient relationships.
• Comply with licensure requirements, applicable certifications, and agency policies.
• Document care efficiently and coordinate with the care team.

As a Home Health LPN, you'll provide skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse. These nursing activities are limited to those in compliance with the state’s/province’s LP(V)N/RNA Practice Act, any applicable licensure/certification requirement, and agency policies and procedures. .

MinimumQualifications
