Job summary

Trinity Health At Home – Muskegon (Michigan) is seeking a Home Health LPN to deliver skilled nursing care under the plan of care prescribed by physicians and the supervising RN.

Qualifications

• Graduate from an accredited or state approved school of practical nursing
• Current licensure in the State of Michigan
• CPR certification required.
• Must have a minimum of one (1) year experience in an acute care setting.
• Home healthcare experience preferred.
• Must have current Driver’s license and reliable transportation to and from work site.

Responsibilities

• Provide skilled nursing care per physician's plan of care and supervisor RN's assignment.
• Practice at the top of your license with greater flexibility, autonomy and 1:1 patient relationships.
• Ensure activities comply with LPN/RNA licensure requirements and agency policies.
• Deliver consistent 1:1 patient care with timely documentation.

As a Home Health LPN, you'll provide skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse. These nursing activities are limited to those in compliance with the state’s/province’s LP(V)N/RNA Practice Act, any applicable licensure/certification requirement, and agency policies and procedures. .
