Trinity Health IHA Medical Group is seeking a compassionate Licensed Practical Nurse (LPN) to support providers in delivering high-quality patient care. You will prep patients, manage results, support care coordination, and assist with medications and procedures to create a positive healthcare experience every day.

The role involves welcoming patients, documenting visits, collecting vitals, assisting with exams, and coordinating care across teams to improve outcomes.

Qualifications

• High School Diploma or GED and graduate of an accredited Licensed Practical Nurse Program.
• Valid, unrestricted LPN license in State of Michigan. Valid CPR certification.
• 1-2 years of clinical experience in a medical or physician office. Previous nursing experience not required.

Responsibilities

• Uses critical thinking skills within the scope of the role to assess if patient information requires higher level follow up.
• Collect and document patient histories, vital signs, and health information.

• Communicate test results and provider instructions to patients.
• Performs general specimen collection duties.
• Provide patient and family education to support better health outcomes.

In this role, you will prepare patients for a provider visit, provide results management support, insurance company advocacy, assist with clinical procedures, support care coordination and gaps in care and medication/vaccine administration in order to help create a positive healthcare experience every day.
