Job summary

Trinity Health At Home is seeking an experienced Licensed Practical Nurse for full-time home health care in Michigan. The role involves one-to-one patient care in patients' homes, using advanced technology and clinical knowledge to monitor and support patients who require dedicated attention.

You will collaborate with physicians, rehab teams, social workers, and home health aides in an autonomous work environment.

Qualifications

• Graduate of an accredited or state approved school of practical nursing.
• Michigan LPN licensure required.
• Minimum five years in acute care; home care preferred.
• Phlebotomy and wound care experience required.
• Must pass NAPNES or equivalent medication exam before administering medications.

Responsibilities

• Provide one-to-one patient care in home setting.
• Utilize technology and clinical knowledge to monitor and support patients.

Minimum Qualifications:

Graduate of an accredited or state approved school of practical nursing, licensure as a LPN in the State of Michigan, a minimum of five (5) years experience in an acute care setting; home care experience preferred. Phlebotomy and wound care experience required. Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
