Bethesda Health Group seeks a Licensed Practical Nurse to provide basic nursing care, assess residents on admission and ongoing basis, and document assessments accurately while communicating changes to the IDT.

You will work at multiple Bethesda locations (Rehab, Long-term Care, Memory Care and Assisted Living), collaborate with families, implement care plans, administer IV fluids, and use SBARR for physician communication; must be able to lift and move residents and perform duties across shifts.

Qualifikationen

• Education from an accredited school of nursing resulting in LPN
• Current LPN license in MO and IL
• Current CPR/AED certification or willingness to obtain within 6 months
• IV certification required
• Two years of experience (three preferred)
• Valid driver's license and reliable transportation

Aufgaben

• Provide basic nursing care and assist in resident assessments on admission and ongoing basis
• Document assessments accurately and report significant changes to the IDT
• Communicate care plans and updates to residents and families
• Administer medications and treatments safely and timely
• Administer IV fluids and monitor responses
• Use SBARR for physician communications and escalate as needed
• Maintain confidentiality and participate in ongoing education and quality initiatives
