Job summary

Hillcrest Care Center & The Towers is seeking a Licensed Practical Nurse (LPN) for full-time evening and night shifts at our Wray, CO location. The role involves direct resident care, medication administration, and supervision of CNAs under a licensed provider’s direction.

Requirements include a Colorado or compact LPN license and BLS certification. A $3,000 sign-on bonus is available for eligible full-time staff with a 1-year commitment.

• BLS certification is required.

Responsibilities

• Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by the nursing assistants under the direction of a licensed healthcare provider.
• Administer medications and treatments, monitor vital signs, and perform wound care as needed.
• Collaborate with the healthcare team and liaise with families.
• Document, monitor, and report patient status and changes.
• Support activities of daily living and provide emotional support.

COMPENSATION BASE RATE

$24.50 to $35.50 dependent on experience (includes shift differentials)

$3,000 sign on bonus available to individuals not employed at Hillcrest Care Center & The Towers in the last 36 months, with full-time employment & 1-year commitment.

Job Description

Provides direct nursing care to residents and supervises the day-to-day nursing activities performed by the nursing assistants under the direction of a licensed healthcare provider in accordance with all applicable laws, regulations, policies and procedures, and Hillcrest Care Center & The Towers standards.
