Job summary

Ascension is seeking an LPN/LVN to provide direct patient care in the CV Surgery department at Ascension St Elizabeth in Appleton, WI. The role requires administering medications, monitoring patient conditions, and educating patients and families on treatment plans.

Schedule is full-time, Monday through Friday, 8:00 AM to 5:00 PM, with supervision under a RN or physician. Applicants should have a valid LPN/LVN license and may use education/licensure in lieu of experience where applicable.

Qualifications

• LPN/LVN license required and within scope of practice.
• High School diploma equivalency or 1 year of relevant experience.
• Ability to administer medications and document responses.
• Ability to educate patients and families on disease treatment plans and post-discharge resources.

What Minimum Requirements You’ll Need

• Deliver direct, compassionate patient care under the direction of a Registered Nurse or physician, ensuring all actions are within the scope of your LPN/LVN license.
• Administer prescribed medications and treatments, meticulously observing and documenting patient responses to ensure accurate, high-quality records.
• Conduct focused patient assessments and skillfully monitor their condition to assure the well-being of every individual.
• Empower patients and families with comprehensive education on their disease treatment plans, self-care, and post-discharge resources to promote sustained health.
• Perform necessary laboratory tests and therapeutic services that contribute to the overall care plan.

Education

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
