Job summary

Incompass Human Services, Inc. is seeking a Licensed Practical Nurse to monitor the health of program participants in our Massachusetts programs.

The role involves providing professional nursing care guidance, managing medical information, referrals, reports, medication administration, and data collection as part of annual planning and intervention strategies. You will conduct medical record audits, coordinate care for individuals with major medical issues, oversee self-medication processes,

Qualifications

• Current LPN license with MA Board of Registration in Nursing.
• At least 1 year experience with elderly or people with developmental disabilities.
• Strong interpersonal, written, and verbal communication skills.
• Valid driver’s license.

Responsibilities

• Monitor health of program participants and provide nursing care guidance.
• Manage and document medical information, referrals, reports, medication administration and data collection.
• Complete medical record audits and review medical tracking systems.
• Coordinate for individuals with major medical issues.
• Provide staff training on medical procedures and emergencies.
• Serve as a mandated reporter and ensure participant rights are safeguarded.
• Communicate with families, clinicians, and DSS for assigned participants.
• Attend meetings and ensure continuity of care between residential and Day Habilitation programs.
