Job summary

North Suffolk Community Services in Winthrop, MA is seeking a Licensed Practical Nurse to oversee health care services for individuals in residential settings. The role ensures regulatory compliance and medical follow-up, supervising CNAs and coordinating with PCPs and specialists.

You will participate in medical discharge meetings, review treatment plans, and provide targeted medical training to staff, including diabetes, nutrition, seizures, oxygen, and vital signs.

Qualifications

• Licensed Practical Nurse registered with the MA Board of Registration in Nursing.
• Minimum of one year of experience in a community health setting.
• MAP certification within five months from date of hire.
• First Aid/CPR certification within five months of hire.

Responsibilities

• Assist with health care services provided to individuals; participate in medical discharge meetings.
• Obtain reports and approval of treatment plans from PCP/Specialists.
• Review seizure and medical incident reports and make recommendations to program staff.
• Participate in managers’, clinical and staff meetings to review health care needs and global health care planning.
• Provide medical training in Diabetes, Nutrition, Seizures, Oxygen and Vital Sign training to all staff.
• Conduct MAP audits at site yearly and follow MAP policies and procedures.
• Perform direct support related tasks in emergencies or as requested by supervisor.
• All other duties assigned by supervisor.
