Job summary

Apple Valley Health & Rehabilitation is actively seeking licensed Practical Nurses (LPNs) to join our long‑term care and skilled nursing team in Yakima County, WA. We offer days, evenings, and weekend doubles with wage rates from $36.00 to $42.02 per hour, plus daily pay access and robust benefits.

As an LPN, you will provide nursing care, administer medications, and work alongside RNs, CNAs, and other professionals to support our residents’ physical and emotional well‑being.

Qualifications

• Current LPN license in Washington.
• CPR/BLS certification (or willing to obtain).
• Experience in LTC or SNF preferred, but new grads welcome.

Responsibilities

• Provide nursing care per standards of practice and orders.
• Administer medications and treatments with precision.
• Collaborate with RNs, CNAs, and interdisciplinary teams.
• Support residents’ physical, emotional, and psychological well‑being.
• Assist with resident comfort.
• Collect samples and monitor catheters; provide wound care.
• Maintain confidentiality and follow all policies and procedures.
• Monitor vital signs and patient condition.
• Coach staff as necessary.

What You’ll Do:

Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

Administer medications and treatments with precision and compassion

Collaborate with RNs, CNAs, and interdisciplinary teams

Support residents’ physical, emotional, and psychological well-being

Assist with resident comfort.

Collect samples and monitor catheters. Provide wound care.

Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.

Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.

Help coach staff as necessary.

Requirements:

• Current LPN license in Washington
• CPR/BLS certification (or willing to obtain)
• Previous LTC or SNF experience preferred, but new grads welcome!
• Positive attitude, reliability, and strong clinical judgment
