Job summary

Hill Valley Healthcare is seeking dedicated LPNs to join our skilled nursing team in a resident-focused environment. You will provide nursing care, administer medications, and work closely with RNs, CNAs, and other professionals to ensure high-quality resident care.

Qualifications

Responsibilities

• Provide nursing care to meet resident needs per standards and physician orders.

• Collaborate with RNs, CNAs, and interdisciplinary teams to support resident well-being.

• Maintain confidentiality and follow all policies and regulatory requirements.

What You’ll Do:

Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

Administer medications and treatments with precision and compassion

• Collaborate with RNs, CNAs, and interdisciplinary teams
• Support residents’ physical, emotional, and psychological well-being
• Assist with resident comfort.
• Collect samples and monitor catheters. Provide wound care.
• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.
• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.
• Help coach staff as necessary.

Requirements:

• Current LPN license in [State]
• CPR/BLS certification (or willing to obtain)
• Previous LTC or SNF experience preferred, but new grads welcome!
• Positive attitude, reliability, and strong clinical judgment
