Job summary

Amedisys is seeking a licensed practical nurse to provide home-based care across Fall River, Westport, Dartmouth, New Bedford, and surrounding areas. This full-time daytime role involves visiting patients at home, performing assessments, teaching self-care, and coordinating with RNs and physicians to ensure optimal care.

Responsibilities include conducting plan-of-care verification, vitals, medication reviews, and timely documentation. Valid driver's license and liability insurance are required.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
