Job summary

AMEDISYS HOME HEALTH is seeking a licensed practical nurse to provide in-home care across Westminster and surrounding towns. The role offers full-time daytime hours with a focus on quality patient care in the comfort of patients' homes.

Responsibilities include verifying plans of care with RNs, collaborating with physicians, teaching patients and caregivers, and performing routine assessments and procedures. A valid LPN license, CPR certification, and reliable transportation are required.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.

• $27.60/hr to $34.50/hr
