Job summary

Amedisys is seeking a licensed practical nurse for full-time day shifts in Indiana to deliver patient-centered care at home. You will coordinate with RNs and physicians to ensure a safe, effective plan of care while supporting patients in their home environments.

Qualifications

• One year of clinical LPN experience.
• Current LPN license for the assigned state.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform procedures ordered by physician (e.g., Foley catheter change).
• Prepare equipment and materials using aseptic technique.
• Complete documentation timely and accurately.
