Job summary

Affirmed Home Care is seeking experienced Licensed Practical Nurses (LPNs) to provide compassionate, one-on-one in-home care in Trenton, NJ and nearby areas. The role offers per diem and visit-based opportunities with flexible schedules and a highly competitive pay rate.

Applicants must hold an active NJ LPN license, have at least 1 year of LPN experience, and maintain current BLS certification. A valid driver’s license and yearly health checks are preferred for field assignments.

Qualifications

Active New Jersey LPN license.
Minimum 1 year of recent LPN experience.
Current BLS certification.
Valid driver's license preferred.
Physical exam within the past year.
PPD/QuantiFERON or chest X-ray as required.
MMR and flu vaccination compliance as required.

Responsibilities

Provide one-on-one in-home nursing care to patients in Trenton and surrounding areas.
Perform assessments, administer medications, and monitor patient conditions.
Collaborate with families and physicians to update care plans.
Per diem and visit-based assignments based on patient needs.
