Affirmed Home Care, Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Newark, NJ.

You will provide one-on-one nursing care to a client in a home setting, following a personalized care plan and maintaining high standards of safety and professionalism. Qualifications

• Active New Jersey LPN license is required.
• 1+ year of nursing experience preferred.
• Current CPR/BLS certification.
• Strong clinical, communication, and organizational skills.
• Reliable, compassionate, and professional.
• Comfortable working independently in a home care setting.
• Valid driver’s license and reliable transportation preferred.
• EHR/documentation experience preferred.

Responsibilities

• Provide compassionate nursing care according to the client’s care plan.
• Administer medications and prescribed treatments.
• Monitor client’s condition and communicate changes promptly.
• Provide wound, catheter, and other skilled nursing care within scope.
• Maintain accurate and timely clinical documentation.
• Promote client comfort, safety, dignity, and independence.
• Communicate professionally with client, family, and care team.
