Job summary

Affirmed Home Care is seeking experienced Licensed Practical Nurses (LPNs) to provide compassionate, one-on-one in-home care in Trenton, NJ and nearby areas. The role offers per diem and visit-based opportunities with flexible schedules and a highly competitive pay rate.

Applicants must hold an active NJ LPN license, have at least 1 year of LPN experience, and maintain current BLS certification. A valid driver’s license and yearly health checks are preferred for field assignments.

Qualifications

• Active New Jersey LPN license.
• Minimum 1 year of recent LPN experience.
• Current BLS certification.
• Valid driver's license preferred.
• Physical exam within the past year.
• PPD/QuantiFERON or chest X-ray as required.
• MMR and flu vaccination compliance as required.

Responsibilities

• Provide one-on-one in-home nursing care to patients in Trenton and surrounding areas.
• Perform assessments, administer medications, and monitor patient conditions.
• Collaborate with families and physicians to update care plans.
• Per diem and visit-based assignments based on patient needs.
