Affirmed Home Care, Inc. is seeking a compassionate Licensed Practical Nurse to provide one-on-one care to clients in their homes around Union, NJ.

This role emphasizes personalized care, reliability, and building meaningful relationships with clients and their families. We offer competitive pay at $45 per hour with flexible scheduling options—full-time, part-time, and per diem—plus health, dental, and vision benefits.

Qualifications

• Active LPN license in New Jersey is required.
• Minimum 1 year of nursing experience preferred.
• Current CPR/BLS certification required.
• Strong communication and clinical skills.
• Ability to work independently in a home-care setting.
• Valid driver’s license and reliable transportation preferred.
• Experience with EHR/documentation preferred.

Responsibilities

• Provide compassionate nursing care based on the client’s individualized care plan.
• Administer medications and prescribed treatments.
• Monitor the client’s condition and communicate changes appropriately.
• Provide wound, catheter, and other skilled nursing care within your scope of practice.
• Maintain accurate and timely documentation.
• Support the client’s comfort, safety, dignity, and independence.
• Communicate professionally with the client, family, and care team.
