Affirmed Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-on-one care in New Brunswick and surrounding Central New Jersey areas. Responsibilities include administering medications, following care plans, wound and catheter care, documentation, and ensuring client safety and independence.

Qualifications

• Active LPN license in New Jersey is required.
• Minimum 1 year of nursing experience preferred.
• CPR/BLS certification is required.
• Strong communication and clinical skills are essential.
• Reliable, compassionate, and professional demeanor for home care.

Responsibilities

• Provide nursing care per individualized care plan.
• Administer medications and treatments as prescribed.
• Monitor client condition and report changes.
• Perform wound care, catheter care, and other skilled tasks within scope.
• Maintain accurate documentation in patient records.

What You’ll Do

• Provide compassionate nursing care based on the client’s individualized care plan

• Administer medications and prescribed treatments

What We’re Looking For

• Active New Jersey LPN License

• 1+ year of nursing experience preferred

• Current CPR/BLS certification

• Strong communication and clinical skills

• Reliable, compassionate, and professional

• Comfortable working independently in a home care setting

• Valid driver’s license and reliable transportation preferred

• EHR/documentation experience preferred
