Job summary

Affirmed Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-on-one care for a client in Newark, New Jersey.

The role offers competitive pay at $45 per hour with flexible scheduling (full-time, part-time, and per diem), benefits, ongoing training, and the opportunity to make a meaningful impact in a home care setting. CPR/BLS certification and an active NJ LPN license are required.

Qualifications

• Active New Jersey LPN license.
• 1+ year of nursing experience preferred.
• Current CPR/BLS certification.
• Strong clinical, communication, and organizational skills.
• Reliable, compassionate and professional.
• Able to work independently in home care setting.
• Valid driver’s license and reliable transportation preferred.
• Experience with EHR/documentation preferred.

Responsibilities

• Provide nursing care per the client’s care plan.
• Administer medications and prescribed treatments.
• Monitor client condition and report changes.
• Provide wound, catheter, and other skilled nursing care.
• Maintain accurate clinical documentation.
• Promote client comfort, safety, dignity, and independence.
• Communicate with client, family, and care team.

What You’ll Do

What We’re Looking For

• Active New Jersey LPN License

• 1+ year of nursing experience preferred
