Job summary

BayCare HomeCare in Sarasota, FL is seeking a Home Health LPN to deliver compassionate in‑home nursing care in a fast‑paced community setting. The role runs 8:00 AM–5:00 PM, Monday to Friday, with every third weekend.

You will work under RN supervision to administer care, IV therapies, and patient education, building strong caregiver partnerships. Requirements include Florida LPN license, BLS, and an Associate's or Diploma in Nursing; 1 year of LPN experience preferred, especially in home care.

Qualifications

• Proven ability to provide nursing care using the nursing process per plan of care.
• Ability to educate patients and caregivers under RN supervision.
• Strong communication with patients, families and team.

Responsibilities

• Delivery of patient care to homebound clients per plan of care.

• Shift Hours: 8:00 AM - 5:00 PM / Monday - Friday
• Weekend Work: Every Third Weekend

• Home visits in assigned service area: Sarasota, FL

Responsibilities

The Home Health LPN provides hands‑on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:

• Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse
• Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse
• Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status
• Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Required

Certifications and Licensures:

• BLS (Basic Life Support)
• LPN (Registered Nurse) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

Education

• Required Associate's or Diploma Nursing

Experience

• Required 1 year Licensed Practical Nurse
• Preferred 1 year Home Care
