We are looking for a full or part-time Licensed Practical Nurse (LPN) to join our team at Summer Pediatrics, located in North Stamford. Our practice serves a diverse community, and we seek a well-trained, highly motivated nurse who is friendly with children, organized, and eager to help. As a certified Patient-Centered Medical Home, we are committed to providing our patients with the highest standard of care and effectively coordinating their chronic health issues.

Responsibilities:

• Nursing care
• Phone triage
• Coordinate daily huddles when needed
• Administer medications and immunizations
• Complete Medical Forms
• Monitor and report changes in chronic patient
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
