Job summary

Elara Caring is seeking a Licensed Practical Nurse (LPN) to provide high-quality in-home care in Missouri. This part-time, hourly role involves travel to patient homes, with supervision from the Clinical Manager or Branch Director for Personal Care Services.

The ideal candidate holds a MO LPN license, has at least 1 year of acute care or home health experience, and excels in communication and organization while delivering plans of care and safety-focused care in the home setting.

Qualifications

• Graduate of an accredited nursing program.
• At least 1 year of acute care experience.
• Excellent communication and interpersonal skills.
• Reliable transportation with travel ~50%.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow Plan of Care prior to each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and care team.

As a Licensed Practical Nurse LPN, you'll contribute to our success in the following ways:

What is required?

• Current LPN/LVN license, in the state of Missouri


• Graduate of an accredited nursing program


• At least 1 year of experience in acute care ( home care, home health or hospice preferred)


• Strong communication and interpersonal skills


• Ability to manage time effectively and stay organized in the field


• Knowledge of healthcare regulations and patient care standards


• Reliable personal transportation (travel required ~50%)


• Ability to meet health screening and compliance requirements

You will report to the Clinical Manager or Branch Director for Personal Care Services.
