Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients’ homes under RN supervision. You will coordinate with the care team, families, and referring agencies to ensure high-quality, patient-centered services.

Requirements include graduation from an accredited practical nurse program, active LPN licensure, and CPR certification; home health experience is preferred. A reliable driver's license is also expected.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Two years’ experience as an LPN, home health preferred.
• Proof of CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.

About the position:
