Elara Caring is hiring a Licensed Practical Nurse to provide compassionate, high-quality home health care across patient homes. You will follow individualized care plans, monitor progress, and coordinate with the care team to ensure safe, effective outcomes.

The role requires an LPN license, nursing education, and at least 1 year of clinical experience, with travel between patient homes and a valid driver’s license. Competitive benefits accompany full-time employment.

Responsabilidades

• Follow each patient’s individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Teach patients and caregivers self-care techniques.
• Collaborate with the RN Case Manager to update care plans as needs change.
• Document medication regimens and update medication profiles per guidelines.
• Communicate with physicians, patients, families, and the care team for coordinated care.
• Help prevent avoidable hospitalizations and ER visits through proactive care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

As a Licensed Practical Nurse you’ll contribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.

What is Required?

• Current State License as a Licensed Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
