Job summary

Elara Caring is seeking a Licensed Vocational/Practical Nurse to join our home-based care team in Stillwater, OK. You will deliver attentive patient care in home settings and work with RNs and physicians to coordinate plans and ensure quality outcomes.

You will follow individualized care plans, document medications, educate patients and families, and help prevent hospitalizations while providing compassionate service. This is a full-time role with competitive benefits and growth opportunities.

Qualifications

• Current State License as LVN/LPN.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Must be able to lift up to 50 lbs and assist with patient transfers.
• Valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Help patients and families learn self-care techniques.
• Update care plans with the RN Case Manager as patient needs change.
• Document medication regimens and update medication profiles.

• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
