Job summary

Amedisys Home Health is seeking a licensed practical nurse to join our team in full-time days. Earn a $10,000 sign-on bonus and provide patient care in the home setting, collaborating with RNs and physicians to manage plans of care.

Responsibilities include verifying plans, performing visits, teaching self-care, taking vital signs, and documenting accurately. Requires 1 year LPN experience, current LPN license, CPR, and a valid driver’s license, with travel to assigned locations.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state(s) you are assigned to.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify when condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
