Job summary

Amedisys Home Health in Bedford County, Virginia, is seeking a full-time Licensed Practical Nurse for day shifts to provide in-home patient care. You will verify care plans, collaborate with RNs and physicians, teach self-care, monitor vitals, reconcile medications, and assist with procedures during visits.

Requirements include one year of LPN experience, an active LPN license for the state, CPR, and a valid driver's license with reliable transportation and insurance.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify promptly when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
