Job summary

Amedisys is seeking a Licensed Practical Nurse to join our home health team in Portland, Oregon. This full-time float role offers daytime hours, Monday through Friday, with pay ranging from $35.00 to $44.00 per hour and the opportunity to help patients heal where they are most comfortable—at home.

Responsibilities include assessing patients, coordinating care with RNs and physicians, teaching self-care, monitoring vitals, and documenting promptly. Benefits and growth opportunities are available.

Qualifications

• One year of clinical LPN experience.
• Current LPN license for your assigned state(s).
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
