Amedisys is seeking a licensed practical nurse in Kernersville, NC to join our home health team on a full-time day shift. You will verify plans of care, collaborate with physicians and RNs, and educate patients and caregivers in self-care at home. A $7,500 sign-on bonus is available.

Qualifikationen

• One year of clinical LPN experience.
• Current LPN license (state(s) you are assigned).
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Aufgaben

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter changes).
• Prepare equipment and materials for treatments using sterile technique.
• Complete documentation timely and accurately.

• Complete documentation timely and accurately.
