Job summary

Traditional Home Health Care, serving pediatric and adult patients in Hazleton and surrounding areas, seeks a compassionate Licensed Practical Nurse (LPN) for full-time and part-time roles. You’ll provide one-on-one in-home nursing, administer medications, assess patients, and coordinate with the Clinical Manager to ensure quality care.

We offer weekday and weekend shifts, competitive hourly pay ($27-$32), direct deposit, PTO from day one, health/dental/vision coverage, 401k with employer match,

Qualifications

• LPN license required and in good standing.
• Experience in home health or pediatric/adult care preferred.
• Ability to work independently and coordinate care.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
