Job summary

StoryPoint in Granger, IN is hiring a Licensed Practical Nurse (LPN) for full-time night shifts from 7pm to 7am. The role offers a $5,000 sign-on bonus and comprehensive benefits with PTO.

You will lead, manage, and train a team of caregivers, evaluate up to 133 residents using EHR/EMAR, and communicate with physicians, residents, and families to maintain high-quality geriatric care.

Qualifications

• LPN in Indiana state license required.
• Assisted Living/Memory Care experience preferred.

Responsibilities

Must haves from you to be a LPN:

• LPN in the state. Assisted Living/Memory Care experience preferred.
• Passionate about working with the elderly.

What your day to day looks like as a LPN:

• Lead, manage, and train your team of caregivers.
• Responsible for evaluating up to 133 residents and reporting change in condition to physicians.
• Utilizing Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR)
• Communicating effectively to team, residents and families.
• Building strong relationships.
