Job summary

StoryPoint Group in Fort Wayne, IN is seeking a Licensed Practical Nurse to lead nursing care for residents in assisted living/memory care settings. You will evaluate residents, coordinate with physicians, and document in EHR/EMAR, while supervising caregivers and maintaining compassionate, patient-centered care.

This full-time role offers day and night shifts, a $33 hourly rate with a $2 night shift differential, full benefits and PTO for full-time staff, and a commitment to delivering

Qualifications

• Licensed Practical Nurse (LPN) in the state.
• Assisted Living/Memory Care experience preferred.
• Passionate about working with the elderly.

Responsibilities

• Lead, manage, and train your team of caregivers.
• Evaluate up to 133 residents and report changes in condition to physicians.
• Utilize Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR).
• Communicate effectively to team, residents and families.
• Build strong relationships with residents and families.

Must haves from you to be a LPN:

• Licensed Practical Nurse (LPN) in the state. Assisted Living/Memory Care experience preferred.
